Seymour Community School District’s pre-employment protocol and paperwork is listed below and must be completed before the employee's start date.
Pre-Employment Background Check: The School Board requires all employees to have a background check prior to hiring. This form will ask about prior convictions, pending charges and suspensions.
Pre-Employment TB Test/Physical: State law requires all school employees to complete a TB test prior to working with students. Employees can choose to make an appointment with their own doctor or go to the Bellin Clinic in Seymour. The Bellin Clinic in Seymour will be paid for by the school district.
I-9 Employment Eligibility Verification: Federal law requires every paid employee to complete the I-9 form by the first day of employment. This includes documentation to verify the individual's identity and eligibility to work.
Payroll Paperwork: Payroll paperwork must be filled out so employees can be set up in the payroll system. This includes the following paperwork: Employee Data Sheet, Direct Deposit Authorization, and W-4.
Work Permit: Per Wisconsin state law, if you are under 16 years old you will need get a work permit prior to starting. It costs $10, but you will be reimbursed by your first paycheck. Please go to the Department of Workforce Development to apply for a work permit.
